Neck and upper back
Neck pain that stops you checking a blind spot, or that arrives with a headache behind the eyes, usually involves the upper joints and the muscles that span them. Arm symptoms change the picture entirely. The examination works out which of those you have before anything is done to the neck.
Reading the pattern
The common presentation is a joint at the top of the neck that has stopped moving well, with the muscles around it tightening to protect it. Rotation drops on one side and the pain sits in a band rather than a point. Gentle mobilisation is often where that starts.
A smaller group has a nerve root involved. That shows as pain into the arm in a defined strip, often with pins and needles or weakness in one movement, and it is tested rather than assumed. The nerve pages describe what the exam looks for.
A third group arrives because of hours at a screen rather than any single event. That is a loading problem, and treating the neck without changing how it is loaded tends to buy a few good days at a time. The plan addresses both or it does not hold.
The examination
Measured both ways against the shoulder line, so the difference is a number rather than an impression.
Each segment of the neck tested for movement and tenderness, to locate the level rather than the region.
Reflexes, grip and sensation, which decide whether this is a joint problem or a nerve problem.
What is proposed for the neck, and what changes at the desk so the same load does not rebuild it.
Honest limits
Neck pain after a significant impact, pain with a severe headache unlike any you have had, visual changes, slurred speech or dizziness on looking up are not treated here. They are screened for at the first visit and they go elsewhere the same day.
Neck pain with fever and stiffness that stops you putting your chin to your chest also belongs with a doctor rather than a clinic, and so does new neck pain in anyone with a history of cancer or significant osteoporosis. Saying so is part of the examination, the full list sits on the red flags page, and no referral is needed to be screened.
Where neck care is appropriate, gentler techniques exist and are used when the screening or your own preference points that way. You can ask for them and the reasoning is explained, which is covered in more detail under safety and in what an adjustment involves.

Anything specific to you belongs in the examination rather than on a page.
Compared how far my neck turned each way and showed me the difference on the second visit. Hard to argue with your own numbers.
I did not want my neck cracked and said so. They used a slower technique instead without making it a discussion.
Four visits, not forty. They said at the start it would be a short run and then it was.
Asked about my desk and my pillow before suggesting anything, which turned out to be where the problem actually lived.
Next step
Rotation measured both ways, each segment checked, and an arm screen before anything is proposed for the neck itself.
